Patient Intake Forms

Upon entering our office, one of our Team Members will welcome you as an addition to our family. Our intake forms can be found in the New Patient Center and can be filled out prior to your appointment; or please plan to arrive 15 minutes before your scheduled appointment to fill them out in office. These forms provide us with your health history and information on your condition. Additionally, a complimentary verification of insurance benefits will be included as we accept most insurances.

Consultation

Next you will have a consultation with Dr. Oien to discuss your health concerns and health goals. Your first visit is designed for Dr. Oien

to learn more about you, your condition and your expectations to determine whether chiropractic care meets your goals.

Examination

After your consultation, Dr. Oien will perform a complete chiropractic examination which can include testing your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic, postural, and physical examinations. Nothing will be done in our office without your consent. In addition, HRV (heart-rate variability) will be included for all patients: this is new technology that tests nerve functionality.

X-Ray Studies

Your specific condition often requires us to take x-rays to get a full evaluation. X-rays help us rule out more serious conditions or assist us in developing the most effective treatment plan for you. Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.

Report of Findings

Once all the information and examinations have been performed and reviewed, Dr. Oien will give you a report of findings and discuss in detail your candidacy in chiropractic care and the care plan that has been tailored to your health condition.
